As an experienced RN, I focus on attentive listening and thorough documentation during phone interactions, building trust and confidence with my patients. I strive to offer the highest standard of care.
I help navigate the complexities of healthcare by connecting patients to community resources, assisting with medical equipment needs, overseeing medication management, and managing transitions of care. I ensure seamless coordination between healthcare providers, including referrals, follow-ups after emergency visits, and post-discharge care.
Patients and providers receive a comprehensive care plan that outlines goals, health conditions, medications, and community services. I ensure easy access to healthcare by providing direct contact with an RN and coordinating transportation assistance for easier access to services.
By partnering with compassionate providers, we deliver team-based care that prioritizes the health and well-being of each patient. I am here to help manage care, answer questions, and ensure that patients have direct access to healthcare professionals from the comfort of their home. Medicare, Advantage Plans and Medicare Supplement Health Insurance are accepted.

Our Founder
Gina Hall is a Registered Nurse with a Bachelor's Degree in the Science of Nursing, a distinction she holds dear as a symbol of her dedication to becoming the best nurse and advocate for her patients.
She is a wife, supported wholeheartedly by her partner, and a proud mother of three wonderful grown children.
Her healthcare journey began in 1995 when her son was born with Down Syndrome. With no prior medical knowledge, she was determined to understand his care better, researching tirelessly and advocating for him to ensure he received the best possible care. This experience sparked her deep passion for healthcare, and she has worked hard ever since to expand her knowledge and skills.
Encouraging her children to pursue careers in the medical field, Gina's son became a Firefighter/Paramedic, and her daughter became a Registered Nurse, both of whom she is incredibly proud of.
Gina’s love for the science of medicine led her to embark on her own career, progressing from Medical Transcriptionist to Certified Nursing Assistant, Medical Assistant, Medical Laboratory Technician, Registered Nurse, and ultimately, Chronic Care Manager over her 27-year healthcare career.
Her son’s care has always been her top priority, so it took time to reach this point, but she’s now ready to share her wealth of experience with her patients.
Gina is dedicated to educating her patients, making them feel comfortable, informed, and supported as she advocates for their health. She’s here to guide them through their healthcare journeys, connect them with vital community resources, and provide continuous support.
